How to apply for a duplicate military parachutist licence and personal jump booklet

Have you lost your military skydiving licence and/or jump booklet? Don't worry, just submit a request directly to CAPAR (Parachute Training Centre, former SMIPAR) in Pisa and you will have your duplicate.

Let us now see case by case the procedure to follow (we have 3 options).

Option 1: Patent card and personal throw booklet

1. Application on plain paper specifying (you can download a facsimile by clicking here);
2. Copy of valid identity card and tax code;
3. No. 3 passport-size photos of which no. 1 authenticated at the municipality of residence;
4. Copy of the report of loss to the competent authorities (Carabinieri or Police);
5. Copy of bank transfer receipt:
   • AMOUNT: 28.66€ which includes:
     • € 16.00 revenue stamp for each 4 sheets produced (excerpt);
     • € 5.00 patent card and personal throw booklet cost;
     • € 7.40 reimbursement by registered mail with return receipt:
     • € 0.26 per sheet produced;
   • ENDORSED BY: COMMAND BRIGADE PARATROOPERS FOLGORE
   • IBAN: IT17 C076 0113 9000 0001 2882 577
   • CAUSE: Issue of duplicate patent card and personal throw booklet of (own name and surname);

Option 2: Patent card only

1. Application on plain paper specifying (you can download a facsimile by clicking here);
2. Copy of valid identity card and tax code;
3. No. 3 passport-size photos of which no. 1 authenticated at the municipality of residence;
4. Copy of the report of loss to the competent authorities (Carabinieri or Police);
5. Copy of bank transfer receipt:
   • AMOUNT: 9.90€ which includes:
     • € 2.50 patent card cost;
     • € 7.40 reimbursement by registered mail with return receipt:
   • ENDORSED BY: COMMAND BRIGADE PARATROOPERS FOLGORE
   • IBAN: IT17 C076 0113 9000 0001 2882 577
   • CAUSE: Issue of duplicate patent card of (own name and surname);

Option 3: Personal throw booklet only

1. Application on plain paper specifying (you can download a facsimile by clicking here);
2. Copy of valid identity card and tax code;
3. Copy of bank transfer receipt:
   • AMOUNT: 26.16€ which includes:
     • € 16.00 revenue stamp for each 4 sheets produced (excerpt);
     • € 2.50 personal throw booklet cost;
     • € 7.40 reimbursement by registered mail with return receipt:
     • € 0.26 per sheet produced;
   • ENDORSED BY: COMMAND BRIGADE PARATROOPERS FOLGORE
   • IBAN: IT17 C076 0113 9000 0001 2882 577
   • CAUSE: Issue of duplicate personal throw booklet of (own name and surname);
